Role Description The overall purpose of the Documentation Specialist is to analyze, document, organize, and publish policies, procedures, and guidelines for internal Operations Divisions. Key responsibilities include: - Work with internal and external customers to gain acceptance and consensus. - Support efforts to improve content taxonomies and content organization. - Stage and publish new and revised records on approval applications. - Secure cooperation of appropriate levels of authority. - Consult with local content owners to ensure standards, templates, and publishing protocol are followed. - Support content organization and standardization efforts that lead to improved search efficiency and improved usability. Qualifications - One or more years experience with corporate content management, knowledge management, and policy and procedural research, writing, and storage. - Experience with documenting software projects. - Proven ability to be effective in a position requiring a high degree of self-direction, initiative, sound decision-making, and good organizational skills. - Strong customer service focus. - Clear and concise verbal and written communication skills. - Consulting and analytical skills. - Sound inductive and deductive reasoning. - Work with others to analyze information and evaluate results to identify appropriate solutions. - Develop solutions to meet individual and departmental goals. - Strong presentation, interpersonal, organizational, and analytical skills. - Strong technical writing and content skills with the ability to relay complex information. - PC proficiency to include Word, Excel, PowerPoint, or other document and web-based application or tools. Requirements - Bachelor’s degree or equivalent work experience required. - Familiarity with Content Management Systems or Knowledge Management Systems. - One (1) year content management/knowledge management experience in a corporate or business setting. - One (1) year healthcare insurance operations experience. - Aptitude to learn Luminare Health operations and technical terminology. - Ability to work with multiple internal and external customers, senior management, subject matter experts (SME). - Familiarity with emerging content/knowledge organization models and practices in the industry. Role Description The Clinical Documentation Integrity (CDI) Specialist will collaborate with physicians, nursing staff, other patient caregivers, and medical records coding staff to improve the quality, specificity, accuracy, and completeness of documentation of care provided and coded. They will review medical records for opportunities for diagnosis clarification and validity as it pertains to DRG assignment, severity of illness, risk of mortality, and case mix data, as well as clinical indicators (HACs, PSI, mortality, etc.). This role will adhere to all coding and clinical documentation improvement guidelines endorsed by ACDIS and AHIMA, while following AHIMA guidelines and Health Information Associates policies and procedures. Title: Clinical Documentation Integrity Registered Nurse RN - Remote Location: Frisco United States Job Description: Job Summary JOB SUMMARY Responsible for reviewing medical records to facilitate and obtain appropriate physician documentation for any clinical conditions or procedures to support the appropriate severity of illness, expected risk of mortality, and complexity of care of the patient, by improving the quality of the physicians' clinical documentation. Exhibits a sufficient knowledge of clinical documentation requirements, MS-DRG Assignment, and clinical conditions and/or procedures. Educates members of the patient care team regarding documentation guidelines, including the following: attending physicians, allied health practitioners, nursing, and case management. ESSENTIAL DUTIES AND RESPONSIBILITIES Include the following. Others may be assigned. - Record Review: - Completes initial medical records reviews of patient records within 24-48 hours of admission for a specified patient population to: (a) evaluate documentation to assign the principal diagnosis, pertinent secondary diagnoses, and procedures for accurate MS-DRG assignment, risk of mortality and severity of illness; and (b) record in business partner designated CDI tool and/or host medical record system. - Conducts follow-up reviews of patients every 24-48 hours or as needed up through discharge to support assigned working MS-DRG assignment upon patient discharge, as necessary. - Formulate physician queries regarding missing, unclear, or conflicting health record documentation by requesting and obtaining additional documentation within the health record, as necessary. - Collaborates with providers, case managers, nursing staff and other ancillary staff regarding documentation and to resolve physician queries prior to discharge. 2.CDI - Communicates/Completes Clinical Documentation Integrity (CDI) activities and coding issues (lacking documentation, physician queries, etc.) for appropriate follow-up, provider education and DRG Miss-Match reconciliation. - Assists with Provider education, rounding and communication regarding open queries for resolution. - Professional Development: Stays current with AHA Official Coding and Reporting Guidelines, CMS and other agency directives for ICD10-CM and PCS coding. Attends CDI Boot camp, CDI/coding trainings annually and quarterly for inpatient coding. Attends monthly education lecture series (MELS) and all CDI/coding assigned learn share modules as well as any additional required CDI education. - Assist in training department staff new to CDI - Performs other duties as assigned KNOWLEDGE, SKILLS, ABILITIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
