Services include eligibility verification, skills and needs assessments, case management, and progress monitoring. The Career Coach also coordinates supportive services and conducts follow-up to ensure that necessary supports are in place for placement in employment. The target goal for participants served is employment at or above the self-sufficiency wage with job retention tracked for at least one year after exiting the program. Essential Functions: Provide information to potential TANF, SNAP, ABAWD, and NCP participants and verify eligibility for program services. Conduct assessments to determine participant needs. Develop and track employment plans and provide case management for each participant and maintain confidentiality. Coordinate supportive services such as childcare, transportation assistance, and work-related support.
