Outpatient Specialty Coder

• Responsible for critical access hospital coding including emergency department, infusions, Critical Access Hospital Specialty Clinic, professional fees, and Rural Health Clinic • Ensure the timely and accurate coding of medical claims while maximizing reimbursement for services • Receive and review charge entry data from practice sites • Identify and investigate incomplete or missing charges • Abstract clinical information; translates medical documentation into diagnoses and procedural codes while utilizing currently accepted coding and classification systems • Sequence codes according to established guidelines • Thorough analysis and interpretation of medical information, medical diagnoses, coding/classification systems, to ensure accuracy for prospective payment system reimbursement • Maintain current knowledge of coding rules and regulations as designated by the AMA, Centers of Medicare and Medicaid Services (CMS) and other payers • Maintain proficient knowledge of EHR, as well as any other systems to perform job duties • Communicate issues to management, including payer, system, or escalated account issues • Identify medical necessity denial trends and provide suggestions for resolution • May perform other billing functions including claim submission, unpaid claims follow-up, denial resolution • Participation in department meetings, in-service programs, and continuing education programs • Convey professional attitude with patients, visitors, physicians, office staff and hospital personnel • Assure confidentiality of patient information, maintaining compliance with policies and procedures • Perform other duties as assigned
Job Requirements
- High School Diploma or GED - required
- Associate Degree - preferred
- Certified Professional Coder (CPC) or Certified Coding Specialist (CCS) - required
- Three to five years in emergency room coding, infusion coding, specialty clinic procedure coding - preferred
- Two years’ experience with formal coding training - considered
- Knowledge of medical terminology - required
- Critical Access Hospital and/or Rural Health Clinic coding is a plus
- Proficient with Microsoft Office
- Demonstrated strong verbal and written communication skills
- Ability to use multiple client systems and manage competing priorities

Abstracts demographic and coding information accurately and completely Core Responsibilities and Essential Functions: - Codes and abstracts medical records with a minimum of accuracy. - Accurately and completely assigns appropriate ICD-10-CM diagnostic and CPT-4/HCPCS procedural codes to the greatest specificity, assigning the most accurate DRG/APC, when appropriate and in accordance with Official Guidelines for Coding and Reporting and Facility Coding Guidelines, as applicable - Accurately and completely abstracts all required patient demographic data into the EMR - Accurately assigns correct APC - Meets productivity standards - Queries providers, if needed to further clarify code - Manages additional coding responsibilities, contributing to the CFB (candidate for bill) goals, including but not limited to: resolving coding edits and reminders, correcting abstracting and coding issues in a timely manner (1-2 business days) - Completes and routes problem accounts, ready to code, high dollar and other accounts daily to ensure cases are coded as close to goal date as possible - Completes assigned work by goal date - Assists with coding unassigned or backlogged accounts - Accurately capture the surgical CPT code(s) for pre-certification reconciliation for each of the WHS facilities, as needed - Serves as a mentor to new coders - Assist with cleaning up or escalating missing documentation or other work queues - If proficient, assists with outpatient and emergency coding when needed or assigned - Performs other duties as assigned - Complies with all WellStar Health System policies, standards of work, and code of conduct. Role Description The Clinical Coder is responsible for assigning accurate and timely clinical codes to patient episodes of care through analysis of patient record documentation and using the appropriate version of the International Statistical Classification of Diseases and Related Health Problems, Australian Modification (ICD10AM) and applying Australian Coding Standards and relevant State Coding authority advice. - Liaise with Clinicians to clarify documentation in the health record to assist in timely and accurate assignment of codes. - Participate in regular coder clinician communication strategies. - Complete Cancer Registry notifications where required. - Review records as required for PICQ or audits. - Involved with ongoing training and education sessions. - Participate in ongoing audits.
