Alternating weeks. - Looking for a strong team player with a positive attitude who can clearly communicate with patients in person and over the phone. The Opportunity: In hospital setting, accountable for utilizing the nursing process. Responsible for performing patient care, primarily telephone triage. Responsible for coordinating the plan of care for a group of assigned patients and consulting with other members of the health care team when indicated to ensure optimal patient outcomes. This clinical nurse demonstrates proficiency and abides by policies, rules, guidelines and procedures. Functions independently and accountable for utilizing the nursing process. Demonstrates increasing discriminatory judgment in interpreting data and prioritizes appropriately. Initiates actions to improve nursing practice and patient care and unit processes. Serves as a day to day resource to colleagues at unit and department levels. Delegates tasks appropriately. Assumes preceptor responsibilities as needed. Utilizes appropriate hospital resources. Is a member of a collaborative care team who is responsible for providing tele-nursing care to a diverse population of patients across various departments within the SLHS. The following features govern the role: patient advocacy, prevention, telecommunication, proactive continuous monitoring, evidenced-based care, clinical decision support and humane, compassionate and ethical care. Goal is to identify early signs there might be an issue, address it, and reduce complications in this population. In clinic setting: - Provide professional nursing care for clinic patients following established standards and practices. - Provide telephone triage, patient flow, and coordination of patient care. - Schedule emergent need patient appointments, identify learning needs for patients and families, provide advice and recommendations, insert intravenous access with medication administration and duties as assigned based on clinic need. - Delegate tasks to clinical staff in accordance with demonstrated competencies and consideration to Scope of Practice. - Serve as subject matter expert for patient education, assessment, interpretation, and documentation of patient care and testing. - Enter admission orders as directed by provider. - Able to work in a team atmosphere as well as autonomously. - Provide excellent customer service skills and professional demeanor at all times. - May cover Medical Assistant, LPN, or other office duties as needed.
