What You Will Do Work assigned coding workqueues to support timely and accurate billing. Assign CPT, HCPCS, ICD-10, and modifier codes in accordance with coding guidelines and payer requirements. Review clinical documentation for completeness and accuracy; identify missing or insufficient information. Communicate with providers and staff to clarify documentation and coding questions. Perform chart and provider audits on an ad-hoc basis. Identify and report payer-specific coding issues and trends to leadership. Request hospital-based notes as needed to support accurate coding. Support coding-related appeals and follow-up on denials. Serve as a coding resource for providers, clinical staff, and business office teams. A Typical Day Monitor and work coding queues, review medical records, assign codes, respond to emails and in-basket messages, contact payers regarding coding-related denials, communicate with providers, and perform audits as needed. Schedule Full-time, 40 hours per week. Monday–Friday with hours ranging between 7:00 a.m. and 6:00 p.m. Work Environment Remote/Home Office environment. Employee must have a dedicated, HIPAA-compliant workspace with high-speed internet and space for two monitors. Ability to work independently and remain productive in a remote setting is required. Travel Onsite to Charlotte, NC offices as needed or requested QualificationsWhat You'll Bring High school diploma or GED required. Minimum of 3 years of medical experience in a physician practice or hospital setting. 1 year of coding experience required; 2 years preferred. Coding certification required (AAPC or AHIMA: CPC, COC, CCS-P, CCS, or equivalent). Understanding of CPT, ICD-10, HCPCS, and modifiers. Knowledge of payer medical and reimbursement policies. Experience in Ophthalmology and/or Otolaryngology preferred, but not required.

Scope: Facility coder with 2+ years' experience in ED facility coding to include diagnosis and procedure coding. Experience with Meditech a plus but not necessary. Target productivity after ramping up will be 12 CPH. This will be shorter term assignment estimated to be 3-6 months. Schedule requirements are M-F. Times of the day worked can be flexible within reason but will need to be available during regular business hours the first day for initial training. Summary/Objective Under limited supervision the Coder ER reviews medical records and performs coding on all diagnoses, procedures, DRG/APC and charge codes. The Coder ER uses the most accurate codes for reimbursement purposes, research, epidemiology, statistical analysis outcomes, financial and strategic planning, evaluation of quality of care, and communication to support the patient’s treatment. The Coder ER will be charged with maintaining the confidentiality of patient records and procedures. Essential Job Functions Responsible for abstracting, coding, sequencing and interpreting the clinical information from inpatient, outpatient, emergency department, pro fee and clinical medical records. Responsible for the assignment of correct principal diagnoses, secondary diagnoses and principal procedure and secondary procedure codes with attention to accurate sequencing. Utilizes technical coding principals and DRG/APC reimbursement expertise to assign appropriate codes. Abstracts and codes pertinent medical data into multiple software programs and/or encoders. Follows official coding guidelines to review and analyze health records. Maintains compliance with both external regulatory and accreditation requirements, and with State and Federal regulations. Extracts pertinent data from the patient’s health record, and determines appropriate coding for reports and billing documents. Identifies codes for reporting medical services, procedures performed by physicians. Enters codes into various computer systems dependent upon the various clients. Track and document productivity in specified systems, maintain productivity levels as defined by the client. Maintain 95% quality rating Perform duties in compliance with Company’s policies and procedures, including but not limited to those related to HIPAA and compliance. The Department of Radiology at University Hospitals Cleveland Medical Center/Case Western Reserve University is seeking a full-time board certified or board eligible Emergency Radiologist. The selected candidate will join 12 other emergency radiologists. The position provides a flexible work schedule consisting of 10-hour shifts every third week and allows for remote coverage. We will also consider radiologists who wish to work remotely and receive similar salary and benefits. The department is committed to excellence in imaged-based clinical care. Responsibilities include providing diagnostic emergency radiology coverage at an academic medical center with an adult and pediatric Level 1 trauma center. SCOPE: Inpatient coder with 3+ years of experience coding in a teaching/trauma level 1 facility. Required to have a minimal of 3 years of Trauma experience. Project uses Epic and 3M, experience with both systems are required. Schedule is flexible within reason, Monday - Sunday; any time between 6am - 10pm EST.
