Responsibilities The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician. Essential Duties and Responsibilities - Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit - Takes history, examines, determines diagnoses. - Provides written documentation of patient visit, per NCQA standards - Takes patient vital signs, as necessary. - Places case management referrals and communicates with PCP as necessary. - Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis. - Performs all clinical duties while observing OSHA Universal Precautions - Maintains patient confidentiality - Attends required meetings and in-services and participates in committees, as requested - Participates in professional development activities and maintains professional licenses and affiliations In this role you may work with. . . - Teammates - Physicians - Medical Staff - Patients - Caregivers - Agency Nurses - Providers - Vendors Qualifications Required Knowledge, Skills, and Experience - Active/unrestricted nurse practitioner license to practice in coverage states - Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA) - Active BLS Certification - Current enrollment in Medicare/Medicaid - Must maintain a valid driver’s license and good driving record - Outstanding EHR skills Preferred Knowledge, Skills and Experience - Geriatric training/experience - Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public Conditions of this role to be aware of. . . - Adaptability to differing weather conditions and patients’ home/residential environments - Full range of body motion including handling/lifting patients. Manual and finger dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking and stair climbing - Regular lifting/carrying items weighing up to 50 pounds - Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings. Job Duties: - Provide a full range of diagnostic neuroradiology services to AHN facilities including a Level 1 trauma center, level 2 trauma center, Comprehensive Stroke Center, Primary Stroke Centers, and a variety of cancer centers. - Equipment includes state-of-the art imaging technologies, including recently installed 3T MRIs throughout AHN and a photon-counting CT at our flagship hospital, Allegheny General Hospital. - AHN utilizes a range of RAPID products for acute stroke imaging. Icometrix was recently employed for imaging of dementia, epilepsy, and multiple sclerosis imaging. - 24/7 MRIs at two hospitals enable therapeutic decision-making for “wake up” strokes. - Functional MRI at Allegheny General Hospital - Participate fully in a division which supplies services to 14 hospitals and multiple imaging centers. - Involvement in clinical and didactic teaching to the residents and medical students is desired. - Active participation in multidisciplinary conferences. Pharmacist (Specialty) - Remote – MO, CA, TX or FL only Position Purpose: Coordinate and assist in managing and participating in pharmacy services, including dispensing of medications, quality assurance and clinical monitoring activities. - Participate in decisions regarding the pharmaceutical services within assigned scope including: - Assessing the patient's general pharmaceutical needs - Planning for the pharmaceutical services provision - Participating in the overall plan of care - Participating in the overall quality assessment and improvement plan - Receive physician prescription orders and evaluate the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stability - Supervise the filling, receiving, compounding, and dispensing activities involved in providing finished product to the patient - Ensure that the patient chart is complete for allergies, height, weight, current and past medical history, clinical laboratory results and concurrent medications (including over-the-counter or home remedy medications) - Ensure that when necessary, laboratory tests have been ordered and the results are obtained and discussed with the nurse and the physician - Provide patient counseling, education, and monitoring - Ensure safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patient - Determine remaining doses on hand to manage inventory and avoid waste - Provide clinical interventions - Assess responses to therapy, patient compliance, and ongoing review of drug regimens; report and manage adverse events - Assist the physician, nurse and patient with clinical and technical support - Document all communication with physicians, nurses and patients in the patient chart - Assist in the maintenance and upkeep of all necessary paperwork required by state and federal agencies as well as the Joint Commission - Assist the Pharmacy Manager in implementing new procedures - Assist in the training and supervision of Pharmacy Technicians, Pharmacy Interns and other pharmacy personnel - Participate in Performance Improvement Projects - Assist in providing in-service programs to nursing and pharmacy personnel on any aspect of drug therapy - Attend and/or complete education and training programs - Review literature and other materials pertinent to the practice of pharmacy - Complete all competency/skills assessment requirements - Participate in on-call schedule - Perform other tasks as determined by the Pharmacist-In-Charge Job Specific Details: - Ensure patients receive the right medication, know how to use their medication, and address/resolve drug therapy issues prior to dispensing. - Order entry - Patient counseling - Verification of prescription data entry, - Performing drug utilization reviews - Working with clinics to clarify and obtain prescriptions - Excellent verbal and written communication - Strong problem-solving abilities - Analytical thinking, time management and organization skills, adaptable and collaborative mindset. Work location: Remote – MO, CA, TX or FL. Candidate will need to be licensed in their current state where we have a physical AcariaHealth location. Working shift: 8.5 Hour shifts / rotating shift within business hours 8 AM – 9 PM (EST / Eastern), includes a few closing shifts. Some holidays are required when business is open. Education/Experience: Bachelor’s degree in pharmacy and 3+ years of mail order, retail or hospital pharmacy experience OR Pharm D. Licenses/Certifications: Current state’s Pharmacist license and license at other states as required. You must be licensed in your home state and that home state must be MO, CA, TX or FL.
