Role Description The Virtual Hospital at West Herts comprises collaboration between West Herts and our community partners at CLCH. The purpose of the virtual hospital is to treat acutely unwell patients in the comfort of their own place of residence, by remotely monitoring them and providing home visits where necessary. Our vision is to support the patient's journey in a way that improves overall patient experience whilst also improving bed flow in the acute wards by preventing admission and supporting early discharge. The role of the Trust Resident Doctor in the Virtual Hospital is to support the daily management, review and flow of patients on the Virtual Ward. Currently, the VH offers the following pathways: - Heart Function (HF) - Asthma, Bronchiectasis and COPD (ABC) - General Medicine - Frailty - Post-Surgical Colorectal pathway Not only do we provide specialist care for our patients, but we work to provide strong wrap-around care to prevent readmissions. The duties of the post will be carried out in conjunction with the Consultant by means of daily ward rounds and follow-up care. The Post holder is expected to undertake appropriate investigations and therapeutic procedures (under supervision). - General ward work and ongoing care of in-patients, including remote ward rounds for inpatients. - Review of patients referred to the virtual ward and face-to-face reviews of inpatients as required, to screen for suitability for VH onboarding. - Maintenance of ward lists. - Attendance to consultant ward rounds. - Attendance at MDTs. Qualifications - Hold for full registration and licence to practise with the GMC - Evidence of completion of Foundation Year 2 competencies - Previously worked in the NHS - Evidence of achievement of Foundation competencies or equivalent in line with GMC Standards/Good Medical Practice - MBBS or equivalent medical qualification - ALS Requirements - Proficiency in written and spoken English adequate to enable effective communication about medical topics with patients and colleagues - OET grade B - Capacity to apply sound clinical knowledge and judgement - Ability to prioritise clinical need - Research skills: Demonstrates understanding of the principles of audit and research Benefits - Flexible working options to support a good work-life balance - Focus on staff wellbeing and development - Opportunities for innovation in improving clinical care, outcomes, and patient experience Company Description With a new hospital planned for Watford, work underway to update theatres at St Albans City Hospital, and plans for a neighbourhood healthcare centre in Hemel Hempstead, this is an exciting time to join us.
