• Serve as the first point of contact for BHCMs on day-to-day operational and care delivery questions • Monitor team schedules, caseload distribution, and daily workflows to ensure consistent patient coverage • Support BHCMs in navigating complex patient situations, escalating to the Director of Behavioral Health and Chief Clinical Officer as appropriate • Promote a culture of accountability, clinical excellence, and psychological safety across the BHCM team • Conduct regular check-ins with individual BHCMs to surface operational barriers and support team morale • Conduct regular shadow sessions with BHCMs, providing structured, real-time feedback on clinical delivery and documentation • Complete chart audits to assess documentation quality, care cadence adherence, and registry accuracy • Identify patterns in quality gaps and escalate systemic issues to the Director of Behavioral Health • Support rollout and adherence to updated clinical workflows and SOPs as developed by the Manager of Education, Curriculum & Credentialing Registry Review Support • Attend weekly registry review meetings led by the Sr. Manager of Psychiatry • Capture and follow up on action items from registry review, coordinating with BHCMs to ensure timely implementation • Monitor patient engagement against clinical cadence guidelines and flag outliers for discussion at registry review • Serve as an operational liaison between the BHCM team and clinical leadership, surfacing workflow friction points and improvement opportunities • Support onboarding of new BHCMs in coordination with the Director of Behavioral Health and Manager of Education, Curriculum & Credentialing • Contribute to the development and refinement of clinical operating procedures based on day-to-day team experience • Assist in tracking BHCM performance metrics and provide input to the Director of Behavioral Health for performance management processes

• Drive Practice Partnerships • Identify and prioritize target pediatric and family practices • Conduct outbound prospecting through phone calls, email, LinkedIn, referrals, and in-person networking • Schedule and coordinate discovery meetings, practice visits, lunch-and-learns, and leadership presentations • Travel regularly for in-person sales meetings, networking events, and conferences to generate new opportunities • Own Pipeline Development • Maintain a disciplined sales pipeline within HubSpot • Track outreach, meetings, next steps, and deal progression • Develop and execute follow-up strategies that move opportunities forward • Report on pipeline health, conversion rates, and key sales metrics • Help Shape Triplemoon's Go-To-Market Strategy • Partner with leadership to determine where we should focus business development efforts • Evaluate conferences, sponsorships, physician dinners, referral partnerships, digital communities, and other growth channels • Identify opportunities to improve messaging, targeting, and sales processes • Provide market intelligence and customer feedback to leadership • Test new business development approaches and help create repeatable playbooks • Represent Triplemoon • Serve as an ambassador for Triplemoon's mission and clinical model • Communicate our value proposition clearly to physicians and practice leaders • Build trust with prospective partners through professionalism, responsiveness, and strong relationship management

• Own the strategy, design, implementation, and continuous improvement of Triplemoon's information security and compliance program. • Ensure ongoing compliance with HIPAA and healthcare security best practices. • Lead readiness efforts for future SOC 2 certification and other security frameworks as needed. • Develop, maintain, and document security policies, procedures, and controls. • Coordinate security incident response, investigation, remediation, and post-incident reviews. • Support customer security questionnaires, audits, and compliance requests. • Partner with leadership to identify, assess, and mitigate information security risks. • Manage and oversee an IT MSP or MSSP who can implement security controls and compliance within SaaS vendors and IT systems. • Provide tiered end-user support for hardware, software, and SaaS application issues. • Provide device and asset management. • Manage identity and access, including systems for onboarding and offboarding. • Maintain system documentation, operating procedures, and technology standards. • Recommend and implement improvements to strengthen security, scalability, and user experience. • Conduct security reviews of third-party vendors and software platforms. • Maintain required security documentation, including BAAs, DPAs, SOC reports, and related compliance artifacts. • Monitor vendor compliance and support periodic risk assessments.

• Set overall vision for Triplemoon’s psychiatry function • Serve as owner of the Company's affiliated professional corporation(s) including all hiring, training, and oversight of Triplemoon’s psychiatry medical staff including psychiatric advanced practice providers. • Support quality assurance • Support care planning development through the creation of structured psychiatric medication management recommendations and guidelines • Oversee professional development of Triplemoon’s broader clinical team including serving as a primary culture carrier and role model of our pediatric care model • Collaborate with behavioral health care managers and PCPs to review patient caseloads, providing treatment recommendations for patients with depression, anxiety, ADHD, bipolar disorder, and other behavioral health conditions • Conduct weekly systematic caseload reviews to monitor patient progress and adjust care plans • Provide indirect psychiatric consultation on diagnostic clarification, psychopharmacologic treatment options, and complex cases • Ensure adherence to measurement-based care (e.g., PHQ-9, GAD-7, other standardized tools) • Participate in the development and delivery of clinical training tools for clinical staff as well as the delivery of educational programming for physician partners • Be available for ad-hoc PCP consultation at the PCP’s convenience and support clinical relationships. • Assist in creation of PCP-facing educational content around psychiatry management in the primary care setting • Document consultations and recommendations clearly in the Triplemoon electronic health record (EHR) • Participate in team meetings, case conferences, and ongoing quality improvement efforts • Stay current with best practices and evolving guidelines in integrated behavioral health • Represent Triplemoon’s psychiatry function at industry events as directed by the Company.

Position Summary The Patient Success Representative plays a key role in supporting patient access to care and ensuring a positive experience across the continuum of services. This position focuses on proactive outreach to patients, assisting with technology and portal access, facilitating care coordination between patients and providers, and connecting patients with community resources. This role is ideal for a people person who has experience and passion for working with families. Backgrounds could include bachelors prepared social worker, nurse, doula, educators, advanced care planners, etc. Key Responsibilities Patient Outreach & Scheduling - Contact patients via phone or secure messaging to schedule and confirm appointments. - Educate patients on Triplemoon services, articulate support services and answer patient questions accordingly. - Ensure patients have completed all necessary legal documents ahead of first appointment. - Follow up with patients who miss appointments or need support rescheduling. - Track outreach efforts and maintain accurate documentation in the electronic health record (EHR). - Solicit ongoing feedback from patients to gauge satisfaction with Triplemoon experience. Technology Support - Assist patients in setting up and accessing telehealth platforms, patient portals, and other digital tools - Provide troubleshooting support and clear instructions to ensure successful use of technology. Care Coordination - Attend weekly registry review sessions and accurately document and follow up on coordination needs that result from these sessions. - Serve as a liaison between patients, clinic staff, and providers to ensure continuity of care and sharing of records. - Coordinate referrals, obtain records, and relay information between patients, Triplemoon clinical team, and external clinical teams. Patient Education & Resourcing - Support Patient Success Lead in the creation and maintenance of patient-specific resource lists based on needs (e.g., transportation, housing, mental health, financial support). Proactively engage patients to provide educational resources and tools in coordination with the patient’s Behavioral Health Care Manager BHCM. - Identify social determinants of health barriers and connect patients with appropriate community resources. - Empower patients to actively participate in their care plan. - Maintain up-to-date knowledge of local and online resources to provide accurate guidance. Documentation & Compliance - Document all patient interactions, resource referrals, and coordination efforts according to organizational policies. - Accurately document all time spent on patient care. - Ensure compliance with HIPAA and other applicable privacy regulations. Qualifications - Education/training: Bachelor of Social Work (BSW), Associates degree in nursing (ADN), Doula certification, education background preferred. - Experience: Candidates should have 3+ years experience in a virtual patient-facing role working in patient engagement, care coordination, social services, and/or healthcare services. The Role Behavioral health care managers play a central role in the collaborative care model. As the main point of contact for each patient and their family, you would both coordinate care and leverage evidence-based behavioral interventions to improve patient outcomes. You would use a process of evidence-based screens, patient education, inquiry, support and personal discovery to build the client’s level of awareness and responsibility. In addition, you'd provide the client with structure, support, feedback, resources, and facilitate access to highly trained experts, as needed. Responsibilities include - Screening: Clinical screening: evaluate results of clinical screens and adapt coaching approaches and techniques to best support the patient’s diagnosis, symptoms and needs. Utilize registry within Triplemoon systems to measure impact over time. - Educating: Host regular, virtual, one-on-one sessions with parents and/or pediatric patients to address concerns related to behavioral health, wellbeing and parenting. - Establishing goals: Empower families to identify and achieve appropriate goals to address their family or parenting challenges. - Listening Empathetically: Listen actively and empathize deeply with members - Applying Behavioral Interventions: Engage patients and/or family in treatment.
