Clinical Pharmacist

Role Description As a Clinical Pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians. You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes, and support the improvement of access to healthcare. - Resolve medication-related queries - Conduct polypharmacy medication reviews and SMRs - Perform medication reviews - Help to utilise skill mix - Improve patient outcomes - Ensure better access to healthcare - Support practice staff with prescription and medication queries - Conduct medicine reconciliation and review post-discharge - Perform medical-related audits and work on service improvement with the practice - Provide expert advice on medications to other clinicians and patients - Deliver clinical leadership on medicines optimisation and quality improvement - Ensure appropriate and efficient use of medication - Educate patients on medication and disease prevention - Understand QOF, CQC, and local incentive scheme workstream - Create and follow clinical pharmacy programme according to practice policy Qualifications - A minimum of 12 months experience of working within general practice/primary care - A relevant degree/qualification to show you are a registered Clinical Pharmacist with GPHC registration - Independent prescribing qualification - Experience with EMIS or System 1 - CPPE Pathway complete or proof of exemption - Excellent communication and interpersonal skills Requirements - IP Qualification - CPPE Pathway - DBS Benefits - GPhC fees and DBS Applications paid - 5 weeks of annual leave plus bank holidays (Pro rata as and when required) - Sick pay (In line with company policy) - Health & wellbeing programme with the following free services: confidential counselling, digital gym, high street stores discounts - An established network of over 200 pharmacists nationally and dedicated senior pharmacist support available - Company equipment e.g. laptop / smartcard reader - Flexibility - Pension scheme

Role Description This is a field-based, national role reporting directly to our Directors. You'll be the clinical voice in our commercial conversations meeting with practices and PCNs, identifying where our teams can make a difference, building business cases, and developing the relationships that underpin sustainable service delivery.
