• Provide one-to-one, person-centered support—at home, in the community, or remotely • Support daily decision-making and help individuals make informed choices in all areas of life • Assist with daily living tasks including personal care, meals, transportation, housekeeping, and laundry • Maintain a safe, comfortable home environment that supports personal goals • Teach and build skills in community participation, health and wellness, safety, communication, social interaction, and household management • Promote problem-solving and self-advocacy through education and real-life opportunities • Administer and document medications and provide required health-related supports • Encourage positive behavior and follow individualized behavior support plans • Respond to crises using prevention, intervention, and safety techniques; report and document incidents as required • Participate in team meetings and complete all required training on schedule • Document services and progress accurately, including daily notes and billing times • Manage a caseload of 20+ individuals with professionalism and consistency
