Care Coordinator - Family Support Department: Serefin Health Employment Type: Full Time Location: Remote Compensation: $60,000 - $65,000 / year Description During the transition and program changes in the new Ontario Autism Program (OAP), parents/caregivers will require essential support and guidance. AccessOAP, as a crucial partner, will directly assist families by providing comprehensive information, orientation, and practical assistance to access the newly available supports, pathways, and programs. Moreover, AccessOAP will work closely with families to determine their child/youth's strengths, needs, and priority goals, utilizing a well-defined process. This dynamic role necessitates individuals who possess creativity, flexibility, solution-oriented thinking, and a collaborative approach. Provide direct support to families who are accessing or are interested in accessing the OAP by helping families: - Understand and make informed choices about their service options in the OAP. - Make informed choices on how to receive funding. - Find and select qualified service providers. - Feel supported in connecting to available resources and services in their local communities. Key Responsibilities - Educate families on all relevant details about the program and ensure that they have a complete understanding of the options and services available to them - Communicate with families in person or on the phone to educate them about the program, to review funding options and guidelines, to provide information about other services and supports - Ensure the “no wrong door” approach to families coming into the OAP through our services and seamlessly working with our inter-agency partners - Ensure that applicant information in the database is current and accurate - Remain knowledgeable and informed on the various program offerings with community partners within your region (Foundational Family Service, Priority Pathways, and more) - Support families to locate qualified, experienced and trained local service providers and refer families to the OAP Provider List to aid in their provider selection as required - Supporting families’ informed choices about next steps for Core Clinical Services - Be the main point of contact for an assigned caseload of families - Support families to locate qualified, experienced and trained local service providers - Develop collaborative relationships and network with related service providers to strengthen external partnerships to help inform families and connect them to programs, services, and resources available in their local communities - Strengthen the capacities of applicants and communities to value and support Autistic Children and Youth by respecting their strengths, competencies, expertise, resilience, and capacities as well as by fostering their inclusion, self-development, and participation within communities Along with, Supporting the organization’s operational goals including other aspects of the business. Skills, Knowledge and Expertise - Diploma or Bachelor's Degree in a relevant field with a minimum of 3 years of experience in human services, social services, or a clinically related discipline, which includes providing support to families of children with ASD and/or developmental disabilities. An equivalent combination of education and similar experience will be considered. - Strong commitment to a family-centered and person-directed approach to support. - Flexibility in engaging with families and adept at navigating challenging conversations with compassion and empathy. - Proven experience working with children and youth on the autism spectrum and their families. - Sound understanding of the challenges faced by individuals with autism, intellectual or developmental disabilities throughout their life course. - Excellent critical thinking skills. - Proficient in conducting interviews and assessments. - Ability to communicate effectively with families using family-friendly language, while also understanding clinical terminology used by other providers. - General knowledge of typical child development. - Thorough understanding of the developmental services sector and familiarity with community-based resources. - Fluency in written and oral French is strongly preferred. Additional languages and experience working with Indigenous families are considered assets. - Demonstrates sound professional judgment and problem-solving skills. - Ability to effectively manage multiple priorities while exhibiting flexibility and accountability. - Proficiency in various computer programs, including database management, email, Microsoft Office applications, and familiarity with social media and other technology tools and approaches. - Detail-oriented with the ability to accomplish administrative tasks in a fast-paced environment with tight deadlines. - Up-to-date police reference check.

Scheduling and Appointment Management Coordinator Department: Serefin Health Employment Type: Contract Location: Remote Compensation: $56,000 - $60,000 / year Description The Scheduling and Appointment Management Coordinator is a supportive role within the Assistant Coordinator team working closely with the care coordination teams and reporting directly to the reporting directly to the Vice president, Serefin Health. Acts as a supportive member working with the scheduling program leads, program management team, care coordinators, and families to ensure the timely management of appointments. This position is dynamic in nature and will require an empathetic and dedicated team player who demonstrates creativity and innovation, while also being caring, motivational, and dedicated. Key Responsibilities - Support the coordination, scheduling, and ongoing management of care appointments with families and clients, including oversight of translation and interpretation services. - Work closely with the program managers to ensure the scheduling practices and protocols are sustained for care coordinators and families. - Leveraging smart technology to ensure all appointments are properly scheduled and care coordinator’s time optimized. - Accountable for managing and confirming all appointments handled are properly managed. - Provide assistance with staff training, education gaps, and on the moment support as it relates to the scheduling and appointment management, as needed. - Assist with developing supporting documents and tools along with developing standardised protocols and policies and procedures as it relates to appointment management. Along with, Supporting the organization’s operational goals including other aspects of the business. Skills, Knowledge and Expertise - Bachelor’s Degree with experience working in a related discipline including providing support to families of children with ASD and/or developmental disabilities with basic understanding of the OAP. An equivalent combination of education and similar experience will be considered. - Sound understanding of issues faced by people with autism, intellectual or developmental disabilities. - Fluency in written and oral English is required and French is strongly preferred. - Good understanding of Microsoft Office, especially Word, PowerPoint, Excel, and Teams along with other software’s required. - Excellent verbal and written communication skills. - Displays ability to navigate through complex relationships, information, and issues, and find solutions. - Demonstrated commitment to principles of social justice, diversity, equity, inclusion, and cultural competence. - Proven, strong problem-solving and conflict resolution skills. - Demonstrated analytical, organizational, and conceptual skills. - Strong work ethic, integrity, and accountability with an excellent attendance record. - Self-directed, courageous, and highly motivated. - Reliable and flexible individual to work as part of a diverse team.
Bilingual Registered Nurse – Care Coordinator, English/Bengali

• Assist patients and their families in understanding healthcare providers' findings, instructions, and advice, keeping the patient at the center of the care plan. • Ensure clinical orders and care plans are executed and up to date to meet the needs of our clients. • Utilize motivational interviewing techniques to communicate progress or deviations to patients, their families, and care providers. • Assist patients and their families in navigating available support resources, including appointment bookings, attendance, and follow-up instructions. • Provide pre-travel advice and guidance to empower clients to travel safely within and outside of Canada. • Offer support to clients with non-emergency questions and concerns while traveling. • Follow up with client’s post-travel to ensure continuity of care upon their return home. • Provide mental health support to patients through coaching and counseling. • Understand crisis intervention and immediate assistance for individuals in need. • Facilitate online therapeutic and counseling sessions addressing various mental health concerns, such as anxiety, depression, stress, and relationship issues. • Consolidate health records, ensuring accessibility for the Care Coordinator, patients, and their families. • Employ patient education and coaching principles to empower patients and their families to actively participate in care decisions, advocating for their unique needs and preferences. • Assist in shaping our evolving health management technology to optimize the patient experience. • Collaborate with other team members to expand our resource library and participate in team meetings to support continuous improvement. • Actively participate in ongoing professional development and training.
