Role Description We’re seeking a trusted, well-connected community member who naturally builds relationships and understands the needs of local families. If friends, neighbors, and community members already turn to you for guidance and resources, this role may be a great fit. As a Community Resource Liaison, you’ll identify and engage individuals and families who may benefit from our Social Care Network Program, with a focus on addressing food insecurity. You’ll share available resources and connect interested individuals with our care coordination team for eligibility screening and enrollment. What You’ll Do - Build relationships throughout the community with individuals, families, and community partners. - Conduct respectful outreach to people who may benefit from additional support. - Share information about available food and social support resources. - Connect interested individuals with our care coordination team for eligibility screening and enrollment. - Help strengthen access to community resources while maintaining professionalism and confidentiality. Qualifications - Is compassionate, approachable, and enjoys helping others. - Has strong ties to the local community and understands the needs of families. - Communicates with empathy, respect, and cultural sensitivity. - Is self-motivated and comfortable working independently. - Has experience in community outreach, healthcare, social services, education, ministry, nonprofit work, or other community-based roles (preferred but not required). Requirements - (1099 Independent Contractor) $25/hour | Flexible Schedule Benefits - Remote position.
