Description Under moderate supervision, responsible for reviewing, auditing, and data entry of medical bills for multiple states and lines of business within both Worker’s Compensation and Commercial Health arenas. This would include analysis for the fee schedule or usual and customary application, as well as PPO interface, while meeting contractual client requirements. Essential Job Functions: - Responsible for auditing medical bills to ensure that they are appropriate and adhere to the State Fee Schedules, customer guidelines, and PPO discounts - Analysis and review of 1 or more assigned states having fee schedules - Utilize Fee Schedules, Online Documents, Client instructions, and other training material to properly review medical bills - Review medical bills for compensability and relatedness to injury - Reprice medical bills to Workers’ Compensation Fee Schedule and PPO Network - Research usual and customary/fee schedule applications and system interface as appropriate - Reviews specialized Medical Bills, which include hospital, surgery, and high-level physician bills for workers' compensation and non-workers' compensation claims, and may include hospital bills, auto liability, and usual and customary reimbursement - Determines the appropriateness of a final reimbursement outcome by making the distinction between and knowing when to apply either Fee Schedule reduction, PPO reduction, Usual and Customary reduction, or Medicare reduction - Communicates and defends to providers and clients the basis for the methodology used to accomplish the reduction of charges - Analyzes and reviews high-level office visits, reports, and record reviews - Interprets hospital review guidelines for both inpatient and outpatient claims - Knowledge of medical terminology, workers’ compensation billing guidelines, and fee schedules, including CPT/ICD/HCPS coding, and knowledge of UB04 and CMS 1500 form types preferred - Responsible for producing a final review for the recommendation of payment to the client - Maintain productivity, as well as speed and level of accuracy, as determined by company standards Requirements Abilities and Competencies: - Current knowledge of utilization review processes and managed care - Knowledge of state-based fee schedules - Strong knowledge of Medical Terminology and CPT/ICD-9/ICD-10 coding - Ability to identify trends through analysis of practices to improve the overall utilization of resources and cost containment - Ability to communicate those trends found through analytical study using a variety of reporting mediums - Ability to work collaboratively and independently while meeting productivity standards - Ability to work in a high-production environment while meeting productivity and quality standards - Ability to represent Utilization Management in organizational committees, as assigned - Excellent relationship management skills - Demonstrated ability to problem-solve in complex situations - Ability to engage in abstract thought - Strong organizational and task prioritization skills - Strong analytical, numerical, and reasoning abilities - Well-developed interpersonal skills - Ability to establish credibility and be decisive – while also recognizing and supporting our organization’s preferences and priorities - Results-oriented with the ability to balance other business considerations - Knowledgeable of multi-state workers’ compensation systems - Computer literacy on Microsoft Office products and database programs - Ability to construct grammatically correct reports using standard medical terminology - Must have a track record of producing highly accurate work, demonstrating attention to detail Education and Experience: - High School Diploma or equivalent - AAPC Coding Certification is required (CPC required, CIC preferred) - ICD-9, ICD-10, PCS/HCPS/CPT, MS-DRG, and Geographical codes, and NCQA regulatory compliance guidelines - Must have a consistent coding rate at the 95th percentile or higher - RAC review and auditing - Proficiency as a Specialty Medical Bill Reviewer with two or more years of previous experience in medical bill review (workers’ compensation is a plus) Driving Essential: No Certifications/Licenses: AAPC Coding Certification (CPC required, CIC preferred) Position Demands: This position requires sitting, bending, stooping for up to 8 hours per day in an office setting. Ability to lift and move objects weighing up to 10 lbs. Ability to learn technical material. Description The Database Administrator (DBA) will serve as the primary database administrator responsible for managing, securing, optimizing, and maintaining the organization’s SQL Server database systems across Azure cloud environments. This role manages a hybrid infrastructure that includes SQL Server on Azure Virtual Machines (IaaS) and Azure SQL Database (PaaS). The DBA will collaborate closely with the data analyst team to support reporting and analytics initiatives and will coordinate with development teams to deploy database changes and application updates into production environments. The ideal candidate is a detail-oriented, mid-level DBA with strong fundamentals in SQL Server administration, a working knowledge of Azure cloud services, and the ability to execute reliably in a production environment. Requirements 1. Database Administration and Maintenance • Configure, manage, optimize, and secure Microsoft SQL Server environments running on both Azure Virtual Machines and Azure SQL Database. • Perform routine database maintenance including backups, integrity checks (DBCC), index maintenance and rebuild strategies, statistics updates, and proactive performance monitoring. • Develop and maintain disaster recovery plans, including backup/restore procedures, geo-replication configurations, and failover testing for both IaaS and PaaS database instances. • Monitor database health and performance using Azure Monitor, SQL Insights, Dynamic Management Views (DMVs), and related tooling; investigate and resolve performance issues. • Maintain database integrity and stability across all environments, ensuring high availability and minimal unplanned downtime. 2. Azure Cloud Database Management • Manage and administer SQL Server instances on Azure VMs, including OS-level patching coordination, storage configuration, and availability group management. • Manage Azure SQL Database instances • Implement and maintain cloud-based data solutions, including database migrations, storage optimization, and environment provisioning. • Leverage Azure resources and services to enhance database scalability, reliability, and cost efficiency. • Participate in capacity planning and make recommendations for infrastructure scaling based on workload trends and growth projections. 3. Application and Database Deployment Support • Execute database deployment scripts (dbdeploy) prepared by the development team as part of coordinated application releases. • Deploy application updates to IIS servers by copying published build artifacts to the appropriate target servers following established procedures. • Validate deployments post-release by confirming database schema changes, application functionality, and system stability. • Maintain deployment documentation, checklists, and runbooks to ensure repeatable, error-free releases. 4. Reporting and Analytics Collaboration • Partner with the data analyst team to support reporting and analytics initiatives, including providing optimized data access, views, and stored procedures for report consumption. • Assist in translating business requirements into efficient SQL queries and data pipelines that feed reporting tools. • Support stakeholders and data analysts by troubleshooting data quality issues, optimizing report query performance, and ensuring data accuracy. 5. Security and Administrative Responsibilities • Enforce security standards and best practices across all database environments, including encryption, auditing, and access controls. • Manage database user permissions, roles, and service accounts; conduct regular access reviews and audits. • Identify and remediate security vulnerabilities proactively across database and related application infrastructure. • Maintain and improve database policies, procedures, and operational documentation. • Continuously evaluate current systems and processes, recommending improvements to enhance reliability, performance, and security posture. Required Qualifications and Skills: • Minimum of 4 years of hands-on experience administering Microsoft SQL Server databases in production environments. • Demonstrated experience managing SQL Server in Azure cloud environments, including both SQL Server on Azure VMs and Azure SQL Database. • Proficiency in SQL Server database administration fundamentals: backup and recovery, performance tuning, query optimization, indexing strategies, and security management. • Advanced SQL scripting skills, including stored procedure development, query optimization, and troubleshooting execution plans. • Experience with database deployment processes, including executing migration scripts and coordinating releases with development teams. • Working knowledge of IIS web server administration, including deploying application files and basic configuration. • Familiarity with Azure cloud services relevant to database administration, including Azure Monitor, Azure Backup, and Azure networking fundamentals. Other Skills: • Microsoft Certified: Azure Database Administrator Associate (DP-300) or equivalent certification. • Experience with Always On Availability Groups and Azure SQL failover groups. • Familiarity with PowerShell and Azure CLI for database automation and administrative scripting. • Experience with data governance practices and enterprise database architecture principles. • Exposure to CI/CD concepts and working alongside development teams using Azure DevOps or similar platforms.
