Role Description The Certified Respiratory Educator (CRE) supports the work of the LHF in delivering programs and services that improve the health of people living with lung disease, as well as in promoting initiatives that help prevent lung disease in the broader community. The CRE plays a significant role in bringing clinical experience and expertise to deliver on the LHF’s mission. - Assist in delivering the My Lung Health Coach program (an evidence-based, person-centered COPD self-management and education program). - Co-facilitate the lung health support group meetings. - Review clinical content including tools, education, and messages. - Requires a licensed provider preferably with a CRE designation. - Work with a team of Lung Health Coaches to create an inclusive proactive approach. Responsibilities - Clinical and Evidence-based champion - Source of clinical knowledge and evidence-based information. - Maintain relevant knowledge and skills related to the role of the Lung Health Coach (CRE). - Assist in external communications and advocacy efforts. - Knowledge Mobilization - Support knowledge mobilization efforts such as needs assessments and environmental scans. - Review curriculum and tools to support behaviour change efforts. - Personalized support visits and engagement efforts - Provide clinical and/or education service for people with lung disease and their caregivers. - Respond to calls, chats, and emails about lung health. - Contribute to the personalized lung health service model. - Provide support in building a patient network across the province and nationally. - Lung Health Foundation Person centered Programs - Maintain a case load of clients within the My Lung Health Coach program. - Promote and co-facilitate the LHF’s virtual support group. - Review drug submissions to Ontario’s Committee to Evaluate Drugs. - Support the data collection process through online surveys and telephone interviews. - Diversity, Equity, and Inclusion (DEI) Skills - Apply a health equity lens to the work they lead. - Support the creation of inclusive materials and programs. - Raise equity considerations in program planning and evaluation. - Foster inclusive collaboration within cross-functional teams. Qualifications - Certified Respiratory Educator required. - Must be a regulated healthcare professional in good standing. - Bilingual in English and French (written and verbal) would be an asset. - Knowledge of Canadian Thoracic Society Respiratory and related guidelines. - Demonstrated knowledge in health promotion, patient education, research, secondary prevention, and chronic disease management. - Demonstrated skills in applying adult and pediatric education principles, project and program coordination, and online learning technology. - Demonstrated excellence in interpersonal relationships, critical thinking, organizational and problem-solving skills. - Excellence in written, verbal, and presentation communication, and effective listening skills. - Receptive to new ideas; creative and innovative. - Nimble, adaptable, and accepting of change. - Ability to handle multiple priorities and meet deadlines. - Experience working with diverse clients and communities. - Excellent computer skills – experience with Microsoft Office programs.
