• The Prevention Case Manager (PCM) delivers housing-focused case management services to families at risk of homelessness, primarily through partnerships with schools and churches. • The PCM plays a critical role in DRM’s homelessness prevention efforts by offering short-term, solution-oriented interventions that stabilize housing before it is lost. • This role requires a high level of autonomy, field-based service delivery, and timely coordination with community partners. • Initiates and maintains effective working relationships with DRM departments and external partners. • Represents DRM professionally in all interactions with clients, schools, churches, and other community contacts. • Participates in required meetings, trainings, and organizational events. • Provides timely, housing-focused case management to families who are currently housed but facing potential housing loss. • Conducts assessments to determine financial hardship and eligibility for services. • Gathers critical documentation (e.g., lease agreements, utility bills, income verification) to support accurate decision-making and financial assistance disbursement. • Collaborates with families to develop short-term housing stability plans, including budgeting, employment retention strategies, and community referrals. • Manages a dynamic caseload of school- and church-referred clients, balancing urgency and thoroughness in service delivery. • Navigates sensitive conversations with school staff, church leaders, and clients, requiring political tact and professionalism. • Works effectively with families from diverse backgrounds, cultures, beliefs, and demographics, providing culturally responsive and equitable support. • Coordinates with referral partners to ensure alignment in family support efforts. • Meets with clients in the field, at schools, churches, homes, or community locations, with minimal daily supervision. • Maintains timely, accurate, and comprehensive documentation in DRM’s case management system and HMIS (within 48 hours of client contact). • Submits financial assistance requests with complete, verified documentation in accordance with internal deadlines. • Supports and models engagement strategies for church-based volunteers involved in family outreach. • Attends team meetings, supervision, and case reviews prepared to report on progress, discuss challenges, and explore solutions. • Responds to client and partner communications within two business days. • Remains informed on relevant resources, policy changes, and best practices in homelessness prevention. • Maintains professional boundaries working with state, city, and local government agencies. • Attends and completes all trainings in a timely manner. • May provide collaborative support to other case management programs and cross-functional projects.
