Role Description Rapid Response/Urgent Crisis Response Clinician Band 6 (Newly qualified Band 5 Nurses will be considered for this role however must have previous community experience) - Work as part of a team of clinicians, therapists, rehabilitation assistants, and Community Geriatricians, prioritising avoidance of unnecessary hospital admission. - Assessment and treatment of patients during a crisis, identifying complex and specialist needs and developing, implementing, and evaluating programmes of care. - Facilitating early hospital discharge for patients back to their own home. - Support and promote the ongoing development of all care delivery within the patients' own home, enhancing the quality of care delivered. - Work with a high level of autonomy, managing undifferentiated and undiagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis. Qualifications - Registered General Nurse/Paramedic with current NMC/HCPC registration. - Completing of clinical reasoning and physical assessment. - Willingness to undertake Independent (V300) prescribing study (desirable). Requirements - Significant post-qualification experience. - Experience of working within the community setting. - Leadership course and evidence of changing practice (desirable). - Experience of managing staff (desirable). - Evidence of advanced clinical skills, including physical assessment, decision making, and psychological therapies. - Communication skills. - Capable of working and making decisions under own initiative. - Administration of IV antibiotics and IV diuretics within the community setting. - Car driver, current UK licence, and access to a car for work purposes. - IT skills. - Flexible to meet the needs of a 7-day, extended hours community service. Benefits - Working as part of the Rapid Response team, providing assessment and treatment of patients within their own home during a crisis. - Identifying patients suitable for virtual ward monitoring and fitting appropriate technology following training and support from the team. - Assessing unwell patients at home on request of the virtual ward hub. - Administering IV antibiotics and IV diuretics in the community.
