Role Description This is a player-coach role. Roughly 60 to 70 percent of your time is hands-on engineering: writing Terraform, designing pipelines, troubleshooting production, making architectural calls that keep our customers running. The other 30 to 40 percent is leadership: mentoring the existing DevOps team, partnering with engineering leaders, building the operational foundation that lets us scale from where we are today to a 5 to 10 person infrastructure team. You will be the senior DevOps voice in the room. You will own decisions about how we deploy, how we secure, how we scale, and how we control cloud spend. You will not need three layers of approval to do your job. What You Will Do - Lead the DevOps function as a player-coach, balancing hands-on technical contribution with team leadership and mentorship. - Design and operate our deployment automation, from GitHub through to customer environments, using Terraform and GitHub Actions. - Drive the active migration from Azure-heavy infrastructure toward an AWS-forward future state, while keeping current Azure workloads stable. - Set the standard for infrastructure as code. Target 95 percent IaC coverage within 12 months. - Own cloud cost. Reduce Azure spend by 50 percent and AWS spend by 25 percent through architectural and operational discipline. - Bake security and compliance into infrastructure by default. Support HITRUST and SOC 2 control requirements as a built-in capability, not an afterthought. - Move our application architecture toward cloud-native patterns (ECS, Kubernetes) where they make sense. - Build and grow the DevOps team. Recruit, onboard, and develop the people who will scale this function with you. - Partner closely with engineering, security, and product leaders. Be the bridge between infrastructure and the rest of the business. Qualifications - 10+ years in DevOps, infrastructure, or platform engineering, with at least the last 3 years in an active individual contributor capacity. - Production-grade Terraform experience. You can design and operate complex IaC patterns at scale, not just write modules. - Deep multi-cloud experience: AWS as primary going forward, Azure for current state. Strong on at least one, real working knowledge of both. - CI/CD pipeline design and operation, ideally with GitHub Actions. End-to-end automation from commit to customer. - Container orchestration in production (Kubernetes, Docker, Podman). - Linux infrastructure proficiency, including shell scripting. Comfortable in mixed Windows / Linux transitional environments. - Track record managing a small team. - Experience operating in a regulated industry: healthcare, finance, government, or similar. - Strong cross-functional collaboration. - Self-directed. You can read a problem, make a call, and own the outcome without escalating every decision. Nice to Have - Prior software development background. - Jira systems administration experience. - Past experience scaling a DevOps function from a small team to a real organization. - Located in the New Hampshire or greater Boston area. Compensation & Location - Base salary range: $150,000 to $185,000, depending on experience. - Target hire is around $165,000. - Performance bonus per company plan. - Standard health, dental, vision, and PTO. - Remote within the United States. Core Responsibilities (What the Role Does) Utilization Management Leadership - Serve as the primary physician resource for UM decision-making, escalations, and complex case reviews - Provide clinical oversight and guidance for medical necessity determinations - Support concurrent and retrospective review processes as needed Peer-to-Peer & Payer Engagement - Conduct physician-to-physician reviews with payers to support authorization approvals and overturn denials - Partner with UM and Revenue Cycle teams to improve payer communication strategies - Identify payer-specific trends and opportunities to improve outcomes Clinical Documentation & Education - Educate providers on documentation best practices tied to medical necessity and payer criteria - Identify recurring documentation gaps and implement corrective guidance - Support audit preparation and response from a clinical perspective Collaboration & Reporting - Partner with Case Management, UM leadership, Revenue Cycle, Compliance, and Operations - Provide insights and recommendations based on denial trends and utilization data - Participate in leadership discussions related to utilization strategy and risk mitigation Required Qualifications - MD or DO with an active, unrestricted medical license - Board Certified in a relevant specialty - Prior experience in Utilization Management, Physician Advisory, Medical Review, or similar role - Strong understanding of medical necessity, payer criteria, and utilization standards - Ability to communicate effectively with both clinicians and non-clinical leaders Preferred Experience (Strongly Valued) - Experience working with commercial payers and managed care organizations - Prior peer-to-peer review experience with documented success overturning denials - Experience in outpatient, post-acute, or multi-site healthcare services environments - Exposure to healthcare services organizations operating in growth or PE-backed environments Core Competencies (Scorecard Competencies) - Clinical Judgment – Applies sound, defensible medical decision-making - Influence Without Authority – Drives alignment across providers and payers - Communication – Clear, concise, and persuasive physician-to-physician dialogue - Decision-Making – Timely, confident, and data-informed - Business Acumen – Understands the financial and compliance implications of utilization decisions What This Role Is Not - Not a purely academic or advisory-only position - Not a full-time practicing clinician role - Not a compliance-only or check-the-box function This role requires ownership, judgment, and real engagement.
