Role Overview We’re looking for a Clinical Pharmacist Lead to build and lead our medication management function across GDMT and broader population health programs. This is a unique opportunity for a strong operator who starts by directly managing patients, shaping workflows, and defining best practices, and evolves into building and scaling a high-performing pharmacy team. You will define how medication optimization is delivered at Chamber: designing protocols, embedding evidence-based guidelines, and creating scalable systems that drive outcomes across heart failure, hypertension, hyperlipidemia, and other cardiovascular conditions. Key Responsibilities: Build the Function • Stand up pharmacist-led medication management workflows across core programs • Personally manage a panel of patients to establish the clinical and operational gold standard • Design protocols for medication initiation, titration, monitoring, and adherence • Identify gaps in medication access, affordability, and adherence, and solve them in real time Scale the Team • Transition from individual contributor to team leader as the organization grows • Hire, onboard, and develop clinical pharmacists and pharmacy support staff • Define roles, responsibilities, and capacity models for pharmacist-led care • Build training programs to ensure consistent, guideline-driven medication management Drive Performance • Use data and insights to continuously improve clinical outcomes and operational efficiency • Partner with clinical and analytics teams to refine risk stratification and identify high-impact opportunities Clinical Oversight & Escalation • Serve as the escalation point for complex medication management cases • Collaborate closely with cardiologists, NPs, and RNs to coordinate care and adjust treatment plans Quality, Compliance & Systems Building • Embed guideline-directed and NCQA-aligned medication workflows into daily operations • Build scalable systems for prior authorizations, affordability support, and adherence tracking • Partner cross-functionally with product and engineering to integrate workflows into Chamber’s platform What Success Looks Like: • A fully built and scalable medication management function with standardized protocols • High-performing pharmacist team operating at the top of license • Seamless integration of medication workflows into broader clinical programs Requirements: • PharmD required; active pharmacist license in DC, MD, VA, GA, TX • 5–8+ years of clinical pharmacy experience, ideally in cardiology, ambulatory care, or population health • Experience with heart failure management and chronic disease management • Prior experience building or scaling programs • Demonstrated ability to move from individual contributor to people leader • Experience in value-based care, population health, or managed care preferred • Strong operator with a bias toward action, ownership, and continuous improvement Location: This is a remote role but you must be based in the United States with reliable wifi. Role Overview: The Case Manager RN collaborates with the care team to develop and execute patient-centered care plans, ensuring timely care progression, safe transitions, and optimal outcomes. This role is responsible for managing patients across the care continuum, identifying barriers, coordinating services, and driving adherence to evidence-based care pathways. Key Responsibilities: Care Coordination & Progression - Review cases within 24–48 hours of admission/trigger and daily thereafter to support care progression - Partner with providers to develop and execute care plans - Identify and address clinical, social, and system barriers impacting care and discharge Assessment & Planning • Complete comprehensive assessments to identify patient needs, risks, and resource gaps • Develop and coordinate individualized transition plans in collaboration with interdisciplinary teams • Align care plans with evidence-based guidelines and organizational goals Transitions of Care • Facilitate safe and timely transitions, including post-discharge follow-up and referrals • Communicate care plans clearly to patients, caregivers, and care teams • Ensure patients and families are equipped with resources for success post-discharge Utilization & Outcomes Management • Support appropriate utilization of services and adherence to clinical guidelines • Identify high-risk patients and initiate interventions to reduce readmissions • Monitor and document key metrics including LOS, avoidable delays, and care progression Communication & Documentation • Maintain clear, timely documentation of assessments, care plans, and interventions • Communicate effectively with interdisciplinary teams, escalating issues as needed • Serve as a resource on care management processes and regulatory requirements Quality & Compliance • Ensure adherence to organizational policies, NCQA standards, and payer requirements • Participate in multidisciplinary rounds and quality improvement initiatives What Success Looks Like: • Timely care progression with reduced delays and avoidable days • High-quality transitions of care with strong patient and caregiver engagement • Measurable impact on readmissions, utilization, and patient outcomes Requirements • RN compact unrestricted RN license required; BSN preferred • 3–5+ years of case management, care coordination, or acute care nursing experience • Strong clinical judgment and understanding of care transitions • Experience with utilization management, discharge planning, or population health preferred • Excellent communication, organization, and problem-solving skills Location: This is a remote role, but you must be based in the United States and have access to reliable wifi. Chamber is building the intelligence layer for value-based cardiology. Our work sits at the intersection of interoperability, analytics, and care delivery. The Role We are hiring a Senior Product Manager, Data & Insights to own the foundation of how Chamber transforms raw data into trusted, product-ready intelligence. This is a platform-level role at the core of our system. You will define how clinical and claims data are standardized, modeled, classified, and transformed into signals that power clinician workflows, operational dashboards, and performance reporting. You will work closely with Engineering, Data Science, Clinical Leadership, Quality, and Operations to establish shared definitions, governance principles, and a clear roadmap for how our data capabilities evolve. This is both strategic and hands-on. You will think in systems, care deeply about signal integrity, and make pragmatic tradeoffs that move the company forward without overbuilding. What You’ll Do Own Data Foundations • Define and evolve canonical data models, taxonomies, value sets, and crosswalks across multiple data sources. • Drive thoughtful decisions about when to adopt external standards and when to build internal abstractions. • Establish versioning and change management practices that protect downstream product stability. Lead Signal & Insight Strategy • Own the roadmap for risk models, prioritization logic, derived features, and core product signals. • Partner with Data Science and Engineering to improve accuracy, explainability, and clinical relevance. • Ensure signals align with value-based care performance and real-world clinical workflows. Strengthen Quality & Governance • Define expectations for completeness, timeliness, and reliability across the platform. • Introduce pragmatic governance frameworks that increase trust without slowing execution. • Create transparency around signal lineage, monitoring, and performance over time. Advance AI & Automation • Identify where AI and automation meaningfully improve insight generation and operational scale. • Collaborate on training data requirements, evaluation metrics, validation processes, and monitoring. • Balance innovation with trust, ensuring outputs remain explainable and clinically credible. Productize Intelligence • Translate platform-level data capabilities into tangible product features, including decision support tools, prioritization queues, performance dashboards, and predictive indicators. • Partner with Design and Engineering to embed insights directly into real workflows. • Define and measure success in terms of adoption, operational efficiency, and clinical outcomes. What We’re Looking For • 5+ years of product management experience, ideally in data platforms, analytics, AI/ML, or technical infrastructure. • Strong understanding of data modeling, feature generation, and data quality concepts. • Experience leading cross-functional initiatives that span engineering, analytics, and domain experts. • Ability to operate at both the strategic and execution levels. • Comfort working in ambiguity and building structure where it does not yet exist. • Bonus: experience in healthcare, value-based care, FHIR or claims data environments.
