• Provide video-based, comprehensive clinical care that encompasses Acute Care, including pediatrics and mental health • Work regular weekend day shifts (7a-7p Sat & Sun +1 Weekday) • Support physicians with Epic inbox management (labs, imaging, patient questions, etc.) • Educate patients on appropriate treatments and care plans for their health needs • Prescribe medication and refills as appropriate • Drive high-quality care with a focus on patient outcomes • Elevate remote care; provide compassionate and meaningful care • Think on your feet to devise creative solutions to problems that arise or escalate as appropriate • Collaborate with Care Team Members • Perform against challenging goals with a best-in-class team

Job Requirements

  • 5+ years of clinical practice experience as a Family Nurse Practitioner in a Primary Care setting
  • Licensed in FL, NJ, NY, MA and/or hold active NLC
  • Must have min 1 year of experience using Epic EMR
  • Must be currently enrolled, or eligible for enrollment, as a Medicare provider
  • Primary care experience and behavioral health experience, ability to treat both acute and chronic care conditions
  • Should be tech-savvy, proactive, organized, detail-oriented, and have telemedicine experience
  • Clean background and medical malpractice history
  • Willing to work weekend hours, 36 hours per week (7a-7p Sat & Sun +1 Weekday)

Responsibilities include: - Working collaboratively with a board-certified cardiologist. - Evaluation and management of cardiovascular conditions. - Formulation of diagnosis and assessments. - Preparing thorough and accurate documentation of encounters and patient care plans. Essential Job Duties - Complies with the individual governing board for NP/PA rules and regulations. - Complies with all licensing, privileging, and credentialing guidelines. - Performs health assessments including patient history and physical examinations. - Supervises, orders, and evaluates diagnostic tests and incorporates them into patient care plans. - Formulates appropriate diagnoses based upon history, physical examination, and clinical findings. - Plans and provides care in collaboration with the patient and family within the framework of individual governing boards. - Prescribes pharmacologic and non-pharmacologic therapies. - Provides patient/family education related to health and lifestyle status. - Evaluates the effectiveness of the plan of care and interventions by comparing patient responses to planned clinical outcomes. - Coordinates and collaborates appropriately with all members of the health care team through timely, efficient, and thorough documentation. - Participates in the education of health care providers and students through mentoring, precepting, providing in-service activities, and developing presentations. - Stays abreast of current trends in healthcare practice and participates in continuing education to ensure optimal clinical knowledge and effective patient outcome skills. - Develops clinical knowledge through continuing education programs and reviewing professional literature. - Participates in interdisciplinary team meetings and appropriate facility meetings. - Assists with emergent situations and calls. - Participates in hospital rotations and call schedules. - Other duties as assigned per the need of the organization and within scope of licensure. Qualifications - Skilled team members with experience in cardiology.
