Through trusted relationships and community-based support, we partner with health plans and local organizations to help members navigate healthcare, stay engaged in care, and live healthier lives. We are launching a new clinical pilot that integrates physicians, care navigators, and community support teams to create a more coordinated and human approach to care. About the Role We are seeking a Pennsylvania-licensed physician to serve as a consulting clinician for an early-stage care program. In this role, you will conduct initial virtual visits with members to understand their medical needs, barriers to care, and overall health goals. Your evaluation helps set the direction for the member’s care journey. After the initial visit, ongoing support is provided by a care team including Community Health Workers and Licensed Clinical Social Workers, allowing you to focus on clinical guidance rather than longitudinal case management. This role is ideal for physicians interested in innovative care delivery, telehealth, and population health models that extend beyond traditional office-based practice. What You’ll Do - Conduct initial telehealth visits with enrolled members - Assess medical needs, care gaps, and barriers to accessing care - Establish a clear clinical plan and care priorities - Collaborate with care team members to ensure a smooth handoff and continuity of care - Provide clinical insight to help refine program workflows during the pilot phase - Support a patient-centered approach addressing social and medical needs together
Job Requirements

- MD or DO with active, unrestricted Pennsylvania license
- Board certified or board eligible (Family Medicine, Internal Medicine, or related primary care specialty preferred)
- Comfort working with Medicare, Medicaid, or complex populations
- Strong communication skills and ability to build trust quickly in a virtual setting
- Interest in care coordination, health navigation, or whole-person care models
- Comfort working in a startup-like, evolving program environment
- Telehealth experience preferred

The ideal candidate brings deep expertise in CNS drug development, experience engaging regulatory agencies, and the leadership skills required in a fast-paced startup environment. The Chief Medical Officer (CMO) role is focused on a west coast client raising $90M to support neurodegenerative disease drug development. The role will likely start part-time, moving to full time fairly quickly. This individual will need an entrepreneurial spirit, deep clinical trial experience, preferably from a pharma/sponsor perspective, and a mature business presence to successfully engage with internal and external business partners. Experience is oligos and brain shuttle technology is preferred. The following roles and responsibilities entail most of what this role may be asked to do, but will expand based on the individual’s and organization’s needs.
