Patient Navigator

Patient Link

• Support patients directly in navigating benefits applications and engage with the community at our partner clinics. • Enroll patients into specified programs using Link Health’s dashboard. • Communicate professionally and appropriately with the leadership team and Senior Patient Navigators. • Complete all required compliance documentation. • Opportunity to write & publish op/ed, conduct research, and present research at conferences.

  • Proficiency in Spanish is preferred
