This is a part-time contract position (20 hours/week to start), fully remote, supporting patients with chronic conditions using ROMTech’s innovative telehealth and remote monitoring platform. Flexible, remote, potential for more hours as patient volume grows. Candidates must be licensed in CT or FL. Responsibilities Conduct virtual patient assessments and monthly check-ins Develop and manage individualized chronic care plans Review device and patient-reported data; intervene as needed Provide education on disease management, medications, and therapy adherence Collaborate with ROMTech clinicians and support teams QualificationsRequirements: NP with master’s degree and active state licensure in CT or FL 2+ years clinical experience in chronic care, primary care, or related field Telehealth or remote patient monitoring experience Strong EMR, documentation, and communication skills CCM/RPM experience, multistate licensure, experience with older adults or high-risk patients Bilinugal (English & Spanish) required
