We’re hiring a Project Coordinator who will drive the implementation of rural health hubs in Pecos, Ward and Brewster counties. This is a home-based position that will require traveling within the three counties mentioned. The ideal candidate will live in the Texas Permian Basin. This is a grant-funded, temporary, part time (20 hours/week) implementer position for 2 year. Responsibilities Under the direction of the Community Impact Director, some responsibilities include: - Coordinate the setup and maintenance of self-monitoring blood pressure stations in partnering libraries. - Support the development of community-based health hubs that provide access to tele-health appointments, education and referrals. - Support community assessments and identify project-related material needs. - Support the libraries’ data collection and reporting quarterly and annually. - Coordinate health information distribution with community partners. - Function as the primary internal and external contact and resource for the program. - Promote and expand positive community relations through active outreach. - Recruit, train, and manage volunteers and strategic community alliances to achieve priority community goals. - Assist in organizing/implementing community and culturally appropriate health initiatives and programs throughout the three counties. - Engage and educate the community with regards to blood pressure resources (coordinate and facilitate market workshops, training, etc. with community, and faith-based organizations) and other health-related initiatives as identified. - Regularly evaluate and report on progress towards stated objectives and provide support for achievement of the grant funding deliverables. Facilitate the completion of pre- and post-evaluations. Qualifications Want to move your resume to the top so that you can become a “Relentless force for a world of longer, healthier lives?” Here are some of the requirements: - Bachelor's degree preferred. - 2+ years of experience working with community/public health issues and/or managing community/public health promotion or related educational programs. - Bilingual in English and Spanish with the ability to read as well as write, preferred. - Effective communication, leadership, and interpersonal skills. - Knowledge of the social determinants of health (SDOH) as it relates to behavior modification through self-care activities and policy/system-wide changes. - Experience working with multicultural & rural communities and in community led programs, projects, or events is preferred. - Experience building and working with a volunteer base. - Ability to travel within the defined geography and to work evenings and weekends if needed. • Process contractor lifecycle updates including onboarding coordination, extensions, renewals, rate updates, and offboarding activities. • Ensure all required documentation and approvals are collected, tracked, and properly stored. • Coordinate with HR, IT, Finance, and Legal to confirm operational steps are completed accurately. • Audit contractor records across systems to ensure data consistency and completeness. • Identify discrepancies in engagement details such as dates, rates, status, and approvals, and correct them. • Clean up legacy data and standardize documentation across engagement files. • Validate dashboards and tracking tools to ensure reporting accuracy. • Monitor active contractor timelines and flag upcoming renewals, expirations, or documentation gaps. • Ensure required approvals are in place before status changes are activated. • Maintain structured tracking tools to ensure full visibility into contractor engagement status. • Update and organize program documentation, templates, and trackers. • Ensure engagement files are audit-ready and consistently maintained. • Support process refinement efforts to improve clarity, consistency, and scalability.
